We are pleased to offer a wide range of office hours including many early morning and weekend appointments to accommodate our patient’s schedules.
It is very important that you review our COVID-19 Office Protocol below. Also, please review our insurance and financial policies when planning your appointment.
If for any reason you must cancel an appointment or will be delayed, please notify us as soon as possible. We appreciate 24 hour cancellation notice. Repeat cancellations will result in a cancellation fee.
In case of a dental emergency, call our office number which provides our emergency phone numbers. If ever both dentists are away, there will be a covering dentist.
For your convenience we have a large lot in the back of our office building, which is where you will also find the entrance to the practice.
Our entire office is handicap accessible including a handicap parking space and a beautiful teak wood entrance ramp which facilitates access to the dental office.
We are happy to accept multiple insurance policies as well as various methods of payment. We will also provide you with any information necessary for you to complete your claim form and ensure you are properly reimbursed by your insurance company.
Our office participates with, but is not exclusive to Cigna Dental Insurance. We have many patients though, who are covered by other insurance carriers and we always do our best to maximize their coverage.
After much deliberation, we have decided to end our “in-network Participation” with Delta Dental by beginning of April, 2021. For more information, please refer to our Patient Notice Regarding Delta Dental.
We accept VISA, Mastercard, Discover and American Express. Its is preferred that you pay for your dental care at the time of service unless other arrangements have been made.

We at Bellevue Avenue Dental Associates make every attempt to make ourselves available to meet your needs while providing a safe environment for our patients and staff.
Following is what you can expect to minimize exposure and help you to feel confident when in our office:
Extensive Cleaning: Infection control has always been a top priority in our office, and our exam rooms have always been thoroughly cleaned after each patient and this procedure will continue. We continue to follow the infection control recommendations made by the ADA, the CDC and OSHA and we will continue to monitor these agencies to make sure we are up-to-date on any new guidelines.
Enforcing Social Distancing: We will be encouraging patients to wait in their cars until their appointment. Please call us to let us know of your arrival. You will be brought directly into the treatment room. Our waiting room will only be available for caregivers of the elderly and disabled or for parents of young children.
Modified Schedules: Appointments will be managed to allow for social distancing and deep cleaning between patients.
Patient Screening: We will screen all our patients with Covid-19 screening questions and we will be taking your temperature with an infrared thermometer. We will postpone treatment for any patient who has experienced or been in contact with someone who has experienced the following symptoms within 14 days of the scheduled appointment date: fever, cough, loss of taste and/or smell, flu-like symptoms, difficulty breathing or shortness of breath and fatigue.
Important Further Considerations: If you have preexisting health conditions (as recommended by the CDC) such as diabetes, chronic lung disease or asthma, serious heart conditions, are immunocompromised, or have chronic kidney or liver disease, we recommend and would like to schedule a visit or need to accompany a child or family member for a visit, please call our office as we will handle this request on a case-by-case basis.
Safety: We will ask our patients to wear a mask as they enter the office and to sanitize their hands. We will have masks and sanitizer available if you do not have any and we will bring them out to you before you enter the office. Once in the chair, you will also be asked to rinse out your mouth with hydrogen peroxide prior to treatment.
Staff Screening: All staff members will be screened daily for symptoms of Covid-19 and temperature will be checked.
Protective Equipment: We will look a little different. We will be wearing face shields, face masks, goggles and protective gowns. But, please know, underneath all the equipment we are the same friendly staff you have always known and we look forward to welcoming back our patients, neighbors and friends.
